Referral Process...
The Domus Pacis Family Respite will not screen or select applicants. The Social Workers, Case Managers, Chaplains, Nurses, Doctors, etc. from participating hospitals and clinics, the American Cancer Society's Patient Navigators as well as a growing list of Referral Partners in Colorado, will identify potential candidates and their families to the Domus Pacis Family Respite. The Health Care professionals follow each cancer patient during his or her cancer program, assuring a continuum of care and offer assistance in lining up additional support services. These Domus Pacis Family Respite partners know both the patient's physical and mental health, as well as their family dynamics.
The nomination process is based on a number of factors which include the patient's medical state:
- Can the patient travel and will the patient need only limited medical care? (the respite will not provide chemo therapy or other in-home procedures)
- Does the patient need and want to participate in the offerings of the family respite?
- Does the patient have a family support system who would be ready and willing to join the patient? Are the family dynamics such that this will be a positive experience for all participants?
If the situation is right, then the patient would be referred by the Referral Partner to the Domus Pacis Family Respite organization who would work with the family contact to schedule a stay dependant on the availability of housing.
An eligibility requirement states that a family is going/has gone through a traumatic cancer event. This includes a family whose loved one is in remission or a family whose loved one has been diagnosed with end stage cancer and has limited time. If a family member has passed and the family needs time to rest/reflect, they are also eligible for a family respite. There is no gender, age, religious, ethnic nor socio-economic limitation. The Referral Partners from our various participating hospitals, physician and cancer support groups are the Family Respite's only referral agents.
